Incident Report (IR)
Investigation
A formal written document that records the details of a specific security incident, surveillance observation, or operational irregularity. Incident reports include the date, time, location, individuals involved, nature of the incident, witness statements, actions taken, and any video evidence references. In casino surveillance, incident reports are the primary documentation tool for communicating observations to management, security, regulators, and law enforcement.

Well-written incident reports use objective, factual language without speculation; they are written as though they will be read in court because they frequently are.
